Shoring up Strattera
With market share slumping, Lilly faces uphill climb to revitalize ADHD drug

By John Russell
john.russell@indystar.com

When it hit pharmacy shelves five years ago this month, Strattera was hailed as a breakthrough drug for hyperactivity and inattentiveness -- and a future blockbuster for drugmaker Eli Lilly and Co.

A LOOK AT STRATTERA

• Maker: Eli Lilly and Co.
• Use: The first nonstimulant to treat attention deficit/ hyperactivity disorder.
• Sales: $579 million in 2006, down 13 percent from the peak of $667.7 million in 2004.
• Developed: By Lilly's Prozac research group in the 1980s, originally as an antidepressant.
• Manufactured: Puerto Rico.
• Price: $3.99 for an average all-day dose (wholesale price).
• Approved for use: United States, Canada, Australia and most of Europe. Approval is pending in Japan.
• Major competitors: Ritalin (Novartis and generic makers), Adderall (Shire) and Concerta (Johnson & Johnson).

WHAT IS ADHD?
• Attention deficit/ hyperactivity disorder is a developmental condition that makes it hard for children to pay attention or to control their behavior. Behavior often is marked by squirming, fidgeting, not following instructions, becoming easily distracted, blurting out answers and excessive talking.
• Three percent to 5 percent (or about 2 million) of U.S. children have ADHD, , according to the National Institutes of Mental Health. As of 2003, 2.5 million children ages 4 to 17 were receiving medication to treat the disorder.
• ADHD was first described by Dr. Heinrich Hoffman in 1845. Yet it was not until 1902 that Sir George F. Still published a series of lectures to the Royal College of Physicians in England in which he described a group of impulsive children with significant behavioral problems, caused by a genetic dysfunction, not poor child rearing.

Doctors and analysts saw Strattera as a worthy competitor to Ritalin, Adderall and Concerta, which dominated the attention deficit/hyperactivity disorder market. Some analysts predicted Strattera could ring up annual sales of up to $1.4 billion within a few years, making it one of Lilly's best sellers.
But after a strong launch and high hopes, Strattera has stalled, dragging Lilly's revenues and disappointing investors.
Sales peaked at $667.7 million in 2004, far short of expectations. Strattera's U.S. market share has skidded from 25 percent of the ADHD drug market in 2004 to less than 15 percent in the first six months of 2007, according to IMS Health.

Catherine Arnold, a drug analyst at Credit Suisse in New York, predicted Strattera sales will decline each year through 2011, the only major Lilly drug to do so. Other analysts also have bleak expectations.

"Strattera is probably going to be flat to eroding over the next several years, and it's unfortunate," said Robert Hazlett, a drug analyst with BMO Capital Markets in New York. "It appears to be a good franchise that has run into trouble."

But Lilly says it stands behind Strattera, and hints it will roll out a major campaign this year in an effort to revitalize it.

But some analysts wonder if Strattera can snap back. The ADHD market is competitive. Other drug makers, including Novartis and Shire, are fighting hard for market share.

Meanwhile, critics are sharpening their attacks on doctors and drug companies, saying ADHD is an invented disease, or that physicians are over-medicating children without considering other options.
Lilly says ADHD is a legitimate, serious disease and is actually under-diagnosed around the world.

"Lots of people don't feel this is a real condition, even though we have lots of data and a huge scientific base," said Dr. A.J. Allen, Lilly's medical director for Strattera. "But parents whose kids have ADHD know it's real."

Strattera faces other hurdles. It carries a black-box warning to alert doctors to reports of suicidal thinking in children taking Strattera, as well as a bold warning about the potential for severe liver injury.
And unlike other ADHD drugs, which start working within days,

Strattera usually requires four to six weeks to take full effect -- too long for some parents, who sometimes need to treat a crisis immediately.

"All in all, I think it's a very tough situation for Lilly," said Les Funtleyder, a drug analyst at Miller Tabak & Co. in New York. "It's a difficult challenge to restart a brand that's stalled out. They're fighting an uphill battle."

Much is at stake for Lilly. The company has invested more than a decade of research, and hundreds of millions of dollars, to test and develop Strattera.

Like most of its competitors, Lilly is facing the toughest business conditions in decades, including a coming wave of patent expirations, increasing pressure from insurers to control prices and slowing industry growth. Analysts say Lilly and its competitors need to get more bang from every product.

"This is a tough business, and certainly not getting any easier," Hazlett said.

Lilly declined to provide details on its turnaround plan for Strattera, including whether it will launch a major direct- to-consumer advertising campaign. In 2005, Lilly spent $45 million to advertise Strattera, making it the 38th- most-advertised drug in the U.S., but cut back sharply the next year, spending just $71,355. Figures for last year are unavailable.

Whatever form the marketing push takes, Lilly said it will focus on a few key messages:

ADHD is a "serious and chronic disease," affecting up to 7 percent of U.S. school-age children, and about 4 percent of adults. The condition is marked by poor concentration, inappropriate activity, distractibility and impulsivity. The impairments affect not only school and work, but also a person's relationships with friends and family.
Strattera is the only Food and Drug Administration- approved treatment that works for 24 hours with a single dose (unlike competing products, which work for about eight hours) and is a nonstimulant with no potential for abuse.

Although Strattera works more gradually than rival products, it doesn't wear off as abruptly.

The big challenge, Lilly said, is that stimulant drugs, such as Ritalin, have been around for decades, and doctors are used to how they work.

"The paradigm that's out there is one that's difficult to change," said Steve Schaefer, brand director for Strattera. "Strattera works differently than the stimulants. Clearly, there's room for greater education on our part."

That's a message that investors want to hear.

"If Lilly was able to put some marketing muscle behind Strattera and get it moving again, that would be great," said Linda Bannister, a drug analyst at Edward Jones in St. Louis. "Otherwise, we have very modest expectations for Strattera."

Strattera was developed in the 1980s by the same Indianapolis- based Lilly neuroscience group that discovered Prozac. In fact, Strattera originally was developed as an antidepressant, but was put on a shelf when Prozac rose to prominence.

In 1993, Lilly scientist Dr. John Heiligenstein proposed testing Strattera as an ADHD treatment. The first study was done on adults at Massachusetts General Hospital in 1995, followed two years later by a study on children. Over the years, Lilly has sponsored at least nine major studies on Strattera.

In the meantime, Lilly is continuing to study which types of patients are best suited for Strattera. Symptoms of ADHD often surface during childhood, and by the time the patients become adults, they often have other medical problems, such as anxiety, depression, alcoholism or drug abuse.

But most of the research for ADHD has been conducted on children, not in adults with multiple medical problems. Lilly said it has been studying the drug intensively lately, trying to target it to the right set of patients with multiple conditions.

"When we launched the drug, we didn't have a clear enough picture of who the right patient was," Allen said. "Now we've got a much better sense."

By the way? My cold is getting a little better.

Ok....Last week while walking toward the Fredericton Soup Kitchen, I noticed close to 10 youths shouting at me.

As in the past? I had a little problem? I had the music on in my ears so I couldn’t make out what they were saying so I just walked inside the kitchen.

Once there, I saw someone and told the individual what happened outside?

I said- Am I in trouble again?

The individual quickly answered- Yes, you are in trouble!!!

I quickly shot back- Ok!!! What did I do now???

The person told me that the story on the streets is this - I take pictures of crack houses and give them to the cops!!!

I said - You mean we have crack houses in this City????..lol...

I continued on by suggesting - You mean the Cops from the Fredericton Police Force are so stupid that an address won’t do??? I have to take pictures of the crack house so they’ll know where it is???..lol

The individual was also confuse so how did a false story like this one get started?

Well? A couple of days earlier while chatting with a volunteer?

This police officer walked by me and began chatting with someone.

His name is Yves. He’s an Acadian from the North Shore.

IMG_8872_edited

I tapped Yves on the shoulder and said- We have a good system here! Don’t screw it up!

I continued- I better shut up because I’ll be charge for obstruction!!!

Yves went into the office.

I waited for the cop because I wanted to know about the issue of Quebec cops working in the Force.

cop3

Around 15 minutes later, he showed up and I walked with the guy till the Fredericton Shelter.

We stood outside for 5 minutes and chatted.

He went inside and I went on my way.

Going back for me taking pictures of crack houses.

Someone showed me a picture which there was around 20 house in one picture and one of them was a crack house!!! < Which I won’t show the picture in here >....lol

The story was that I was an informant and I met Yves at the shelter to give him the picture.

I quickly shot back- If I was an informant? Wouldn’t that be stupid of me to give a cop a picture in front of everyone?

The individual replied - Charles?? They think you’re crazy!!!!

Just too funny!!!!...lol

First it was the pedophile story and now I’m a drug informant!!!

I may be crazy but not stupid!!!

I know a lot of people who does drugs and that’s their business.

As a matter of fact, I always try to help them by telling their stories in my blog so we can have more money for methadone.

There’s nothing I can do.

What has to happen if someone confronts me on the streets in a violent matter?

If he beats me?

That’s fine!!!

If I beat him?

That’s fine!!!

I will call the cops and lay charges.

During a court trial?

We might know where or who are starting these stories?

There’s no question that many enemies are reading this blog and wish to do me great harm.

Does this scare moi?

Of course not!!!

I’m just going to continue to do what I do best and THAT’S BLOGGING ISSUES AWAY!!!

It may get more dangerous and I will continue.

By the way? The reason I decided to blog this issue?

If something does happen to me?

I don’t wish the cops to tell the public that Charles took one picture too many!

Truly stay tuned!!!!

I didn't know anything about this one until someone email me this one.

Yeah, someone or group should get together and put this idea to work!!!!

Walking trail club a good idea

I read Charles Leblanc’s letter about wanting the city to clear walking trails and I thought why not start a trail walkers club? It would need someone dedicated to the cause, much like Leblanc, to get it going at first. But in the end it would be a great club with lots of members, and the trails would be full of people all year long.

You could sell memberships and buy groomers as the club expands. Club volunteers could take turns grooming the trails. Walkers could buy a pass, and a tag could be worn to identify club members to the trail wardens something like snowmobilers do now.

Just think of the cash this type of system would save the city for things like providing the greatest amount of services to the largest amount of people within their budget.

SPIRITUAL LIVING
Prayer and Devotions

Answering the Call
by Father Paul Campbell, LC

Resolution: Today, speak to someone about Jesus’ love.

January 19, 2008
Saturday of the First Week in Ordinary Time

Mark 2:13-17
Jesus went out along the sea. All the crowd came to him and he taught them. As he passed by, he saw Levi, son of Alphaeus, sitting at the customs post. He said to him, “Follow me.” And he got up and followed him. While he was at table in his house, many tax collectors and sinners sat with Jesus and his disciples; for there were many who followed him. Some scribes who were Pharisees saw that he was eating with sinners and tax collectors and said to his disciples, “Why does he eat with tax collectors and sinners?” Jesus heard this and said to them, “Those who are well do not need a physician, but the sick do. I did not come to call the righteous but sinners.”

Introductory Prayer: Lord, thank you for this time to be with you, to learn from you, to draw strength from you. I believe in your love. Impress it deeply into my heart. I want to love with all my heart, with all my strength and with all my mind. You know how much I need you. Help me to love you as you deserve to be loved.

Petition: Lord, give me the grace to respond joyfully to your call in my life.

1. Matthew’s Call Jesus took the initiative of finding and calling the men he wanted to follow him. Matthew was sitting at his table doing his work. He wasn’t looking for a vocation. He certainly didn’t ask Jesus to come. Jesus simply walked into his life and turned his life upside down.

2. Matthew’s Response Matthew didn’t ask to be called. However, when Jesus called, he got up immediately to follow him. Matthew didn’t argue with the call or fight it. He accepted Christ’s invitation to participate in his life and mission, and he accepted the changes that this meant.

3. Matthew’s Work After his call, Matthew got to work. He invited friends over to his house so that they could meet Jesus. Matthew doesn’t keep Jesus to himself. He actively spread his friendship and love for Jesus with others. He wanted them to enter into the friendship that he had. Jesus wants to save many souls, but he depends on our efforts to share him with others. Jesus turns no one away. He is not afraid to be associated with sinners. This is the good news of salvation.

Conversation with Christ: Lord, I know that I need to share you with others. Help me. Help me to give them a good example by the way I live my life and speak to others. Help me first to be a witness with my life and then not to be afraid to speak of you and what you have done in my life. I need to remember the number of souls that are lost and adrift in this world. What am I doing for them? What would you want me to do?

Resolution: Today, I will speak to someone about Jesus’ love for that person.
